ATS Physical Therapy welcomes new patients for rehabilitation of the joints and muscles due to injury, disability, illness and motor vehicle injuries. We accept Medicare, Medicaid, and most insurance carriers for physical therapy services. Fill out the following forms before you come into your first appointment and please don’t forget to bring your insurance information, and a picture ID.
